Payment Policy
Self-pay (out-of-network) clients:
I will provide you with a superbill suitable for you to submit to your insurance. The superbill (which will also serve as a payment receipt) will be coded appropriately to the level of service provided during the visit. You agree to pay me at the time of the visit (credit card, or FSA).
Insurance (in-network with TLN or Wildflower) clients:
My initial visit includes 2 weeks of follow up support by secure messaging or email. Continued support is available for a monthly fee of $40 via secured messaging through the client portal. These fees are elective and not eligible for insurance reimbursement.
Using PracticeQ:
Paradise Lactation Care uses PracticeQ to process payments made electronically using a credit card or fund transfer. Payments may be made electronically using a credit card or fund transfer. Stripe is a secure and private platform that meets the high standards of HIPAA and the banking industry. However, Stripe may send, automatically or per your request, email or text message receipts that reveal personal health information such as the date and type of lactation visit.
Cancellation policy:
Please understand that you are responsible for all charges associated with this visit. If you cancel with less than 24 hours notice, your credit card on file will be charged $100. For example if your appointment is on Wednesday at 9:00am I need to hear from you BEFORE Tuesday at 9:00am. You are allowed to reschedule up to 4 hours before your scheduled visit. To avoid this charge you must notify us the day before your appointment. If you need assistance with rescheduling please contact me directly at (352) 801-6025.
Starting September 1, 2024, a travel fee will be required for all home visits.

Travel Fees for ALL clients requesting home visits:
The fee schedule is as follows:
$30 for distances within 20-minute distance from Sumterville.$50 for distances exceeding 20 minutes from Sumterville.$70 for distances between 45 minutes to 1 hour away from Sumterville. Travel fees will be applied to your invoice once you schedule your appointment and will be charged to the card on file on the day of your appointment. This fee is not covered by insurance.
** Please note that home visits are offered on a very limited basis. Follow-ups will be offered via home visit or virtual.

Privacy Policy
Paradise Lactation Care is committed to preserving your right to privacy. Paradise Lactation Care does not collect identifying information about you (such as your name, address, telephone number, or email address) unless you voluntarily submit that information to us through my Contact Me page, my newsletter subscription pages, or other means.
Please do not provide any confidential information (including health information) to us via electronic communication. Any information you do submit will not be sold to any third parties. If I publish any information you provide to us on the website, we will only disclose personally identifiable information agreed upon by you.

Covid-19 Policy
Tenesha Davis is taking every measure advised by the government and medical professionals to prevent the transmission of COVID-19, including the use of Personal Protective Equipment (PPE) and viricidal disinfectants before and after every appointment.
However, this does not completely eliminate the risk of exposure or spread to yourself or those whom you are in contact with. There is ever-evolving information regarding Covid-19, and we will take all necessary precautions as recommended by the CDC.
